Just installing freshly, but from what i saw in endgame save file, combat is real and great. Better Combat Enhanced - great skills/signs and other changes overall.

Witcher 3 enhanced edition(W3EE) - amazing changes but game breaking for me was long/short atack switch and frustrating targeting system, too much accentuated dodging system. Ghost Mod(GM) + Wraith Mod(optional, removes item lvl requirement) - great overhaul mod Wiedzminsky Reshade(great but complex to install) Realistic Contrast Textures(UREAL has it) Ultra Realistic Environment and Light(UREAL)

Optionals(i go with RCT/UREAL, but Compilation is also great) HD Reworked Project(a must upgrades textures and details) Its WIP, dont bitch if you dont like something! Oh wait, this is codex.nvm. Its just based on my first and almost second playthrough and you are welcome to add/suggest/comment. The author has the color scheme exactly right though so you could tweak your own based on that preset. This is a category/list of best mods, depending on their purpose. Althought some think it is too bright and it comes with a ~10-15 fps performance hit which for me isn’t an issue as I still cap out at 75 but it might lower yours too much. Saldax is, imo, the best preset that works with PLM. There are other good lighting mods but I feel they all give too much of a bland/bleak feel to it which is fine for Velen but TW3 isn’t Skyrim and it generally has quite a vibrant atmosphere despite the more darker themes that exist in the series. It is also one of the few that comes with its own weathers. I feel this captures the vanilla witcher 3 feel best and is merely an enhancement of this. PLM can be bright and colorful and has dark nights and generally a lot of darkness where there are no light sources (aka you need to use a torch). It kind of depends on what kind of art style you like. I am using PLM Ultimate v3 with Saldax Reshade.
